Some of them measure blood pressure, others estimate it. And this difference is crucial. Let’s discuss it.

How Do Blood Pressure Wearables Actually Work?
There isn’t one single way these devices get to a number, and honestly, most of them aren’t giving you a real “number” at all.
The majority of smartwatches and smart rings rely on optical sensors, those familiar green or red LED lights used for heart rate monitoring. The principle of action is based on measuring light reflection from blood vessels and based on this, calculating the changes in your blood pressure. This approach uses the principle of photoplethysmography, or PPG, which works silently but efficiently on almost every device on the list.
A few devices, like Samsung and Amazfit, combine the optical information with pulse transit time, which is basically how quickly the pulse wave moves between two points and calibrates the result using the conventional blood pressure cuff once every couple of weeks. Without regular calibration, the results would be drifting away from what is actually happening.
There’s another company taking a completely different approach, Huawei, packing the watch strap with a miniature inflatable cuff for oscillometric measurements.
This is all non-interchangeable data though. The trend analysis on the ring is not the same type of data as the inflatable cuff reading, even if both show up as two numbers on a screen.

1. Apple Watch Ultra 3

Let me make one thing clear so there are no misunderstandings: the Apple Watch Ultra 3 doesn’t measure your blood pressure. It looks for warning signs of hypertension and then advises you to go get checked.
For this it uses the optical heart rate sensor to monitor your blood vessels’ reaction to every heartbeat of yours and after 30 days of quietly collecting information in the background, it will alert you in case anything suspicious is found. This particular feature was developed based on thorough research and is FDA-cleared, giving it more validity than “wellness only” disclaimers from the rest of the list. It’s not for those under 22 years of age and not for those already diagnosed with hypertension.
With its price starting from $799, the Ultra 3 is the most expensive watch featured here but blood pressure tracking is merely one of the features available in this rather large rugged smartwatch. Battery life of up to 42 hours under normal use and up to 72 hours in Low Power Mode is quite a good result for an Apple Watch, while satellite messaging allows staying in contact in places far off the grid. Apple Health app integration is, in my opinion, the best in this segment and all the data is collected and synced automatically the moment you open your phone.

2. Samsung Galaxy Watch Ultra 2

Where Apple gives you an indication of an irregularity in your pattern, the Korean giant, Samsung with its Galaxy Watch Ultra 2, gives you on-demand readings of your systolic and diastolic blood pressure with the help of its BioActive sensor, provided you calibrate it properly once using a real cuff and then recalibrate it every four weeks or so.
The Ultra 2 is a recent release in July 2026 and while many things have remained the same as in the original Ultra, there has been a significant increase in battery capacity, up by about 35% to 800mAh, giving up to 60 hours of battery life. The company is marketing this technology as the Blood Pressure Trend and promoting it together with such features as Antioxidant Index and the recently introduced trail running feature. The Blood Pressure Trend is not an FDA-approved feature and should not be used for diagnostics.
It is priced at $699.99 and is available in a single size of 47mm titanium case. Not the most compact model if you have relatively small wrists. It has a 5,000-nit screen that is extremely bright even in outdoor conditions. It is powered by a new Snapdragon Wear Elite processor that is definitely faster than that in the previous Ultra. And the app does everything which is maybe a little too much for people new to Samsung Health.

It features the same Snapdragon Wear Elite processor as the Ultra 2 does and the new Samsung Vitals app checks 5 overnight biomarkers, such as heart rate, HRV, breathing rate, skin temperature and blood oxygen levels, compared to the individual baseline rather than generic averages.
Prices start at $379.99 for the Bluetooth 40mm version, with $429.99 for LTE and $409.99 for the 44mm Bluetooth version and $459.99 for the 44mm LTE. The 40mm version comes with a 390mAh battery, while the 44mm version gets an extra 445mAh battery and both are said to provide up to 30 hours of use with AOD. Like the Ultra 2, in order to check your blood pressure, you have to calibrate the watch against a cuff once per month and the blood pressure test is a wellness one rather than medical.
In my opinion, the nice thing about this smartwatch is that it offers practically the same health monitoring suite as its much more expensive cousin Ultra 2, but costs almost twice as less, without the unnecessary titanium case and waterproofing. Also, the app experience on the Galaxy Watch 9 and Galaxy Watch Ultra 2 is identical as they come with the same Samsung Health app.

4. Huawei Watch D2

This is the only device from the list which really acts like a cuff since Huawei managed to fit a small inflatable airbag directly into the strap and takes proper oscillometric readings as the device at your doctor’s. It’s the only device from the list certified as a medical device by both EU and China, plus it supports 24-hour Ambulatory Blood Pressure Monitoring (ABPM) just like the clinical-grade ABPM device.
The strap is thicker than the regular watch strap due to the airbag and it takes less than one minute to take a measurement, but still, the strap needs to be inflated properly, so you have to sit still during the process. Battery life lasts up to 6 days depending on the number of readings and ECG measurements per day. By default, it takes 6 BP and 5 ECG readings in a day.
The price of the device is about £350 in the UK. In the USA, it’s not officially available yet. Huawei Health app works fine but lacks some features which are present in Apple Health and Samsung Health. It also supports contactless payments in certain regions but the focus is more on health data than on smartwatch stuff.

The Zepp app aims to cover all bases, including sleep, stress, workouts, blood pressure trends, nutrition, etc., but it can become overwhelming to navigate through, although Amazfit does streamline it consistently with each update.

As the ring detects when you’re not getting consistent blood pressure drops, this plays similar to how Apple detects deviation in your trends.
Speaking of the Oura Ring 5, it got an official redesign in this generation and it’s another big selling point in addition to its blood pressure monitoring. It’s 40% smaller than Ring 4 and measures 6.09 mm in width and 2.29 mm in height. The company redesigned the ring’s sensor architecture, increased the power of the LEDs and added twelve signal pathways for accurate readings regardless of skin color and finger shape. Moreover, the battery life wasn’t affected by this change and remains between 6 to 9 days depending on your preferences and ring size.

Also, to access all of the features of Oura ring, you will have to purchase the Oura membership, which costs $5.99 per month or $69.99 per year because without the subscription, it’s basically just a paperweight. The app experience of the product remains its main strong point and it’s designed to showcase long-term trends in a nice and well-organized manner.

What sold me was the battery life, which is estimated to last up to 14 days per single charge. It is complemented by a charging case which doubles as a reserve, extending the battery of the device to more than 150 days without needing to charge it from the power socket. That is exactly the kind of metric that really matters if you often travel or just don’t like thinking about charging smaller devices.
Priced at $349–$369 with absolutely no subscription, the app is simple, trend data is presented well day-by-day. RingConn has been adding context tooltips to help with vascular load graphs which can get a little hard to decode at times.

This could be a game changer in the category of blood pressure tracking wearables since it doesn’t call for a cuff reading even for calibration but still gives a systolic and diastolic reading.
According to Signal, the ring is precise enough for it to skip calibration and reads blood flow directly and captures changes faster than other blood pressure trackers. It guides users through a “Zen Mode” breathing session each time it takes a measurement while keeping track of their breathing pattern in the background.
At $399, it does not require a subscription to access the measurements and its pre-order is refundable within 30 days with a non-refundable fee of $25. There is no comment on accuracy since it hasn’t been released but this is for sure a wearable I have my eye on.

9. WHOOP MG

WHOOP has always been screenless and built around the membership, not the hardware and WHOOP MG is the tier with the medical-grade sensors on top of standard strain and recovery measurements. Blood Pressure Insights provided by this wearable is different from other wrist-based devices on our list in that it doesn’t provide on-demand readings. You get just one estimate per day, calculated based on overnight sensor data, heart rate, HRV and blood flow patterns during sleep.
To enable this feature, you have to log your blood pressure in the app with the help of a cuff first, establishing your baseline and WHOOP emphasizes the fact that this feature provides you with a wellness estimate, not some kind of medical measurement. Are Blood Pressure Wearables Actually Accurate?
Well, here is an honest answer, and it all depends on what blood pressure wearable you choose.
The Huawei Watch D2 would be closest to an actual measurement as it uses an inflatable cuff and is medical certified in Europe and China. Most other wearables use the calibration procedure based on your cuff measurement and then make estimates based on optical sensors. The calibration procedure is not to be skipped because without it, the trend measurement would be of no use.
None of this could replace a discussion with your doctor.
Final Verdict
What really hit me about all of this isn’t which blood pressure wearable device wins. What hit me is how much work the industry still has to do in defining the very concept of “measurement”. Heart rate was easy enough to define. Sleep stages, too, kind of. Blood pressure has been resisting a shortcut and every company on this list is doing something differently to get around that, from cuff measurement to alerts to tracking trends or dips at night.
As far as I can tell, there isn’t really a right or wrong way to go about it yet.
